The FS has five copies, one original and four copies, two for the waste producer or holder, one for the waste transporter, one for the waste manager and one for the Agència de Residus de Catalunya.

Each box of the document must be completed as follows:

Procedure The FS will be given to anyone who requests it representing any enterprise recorded in the database of the 'Agència de Residus de Catalunya, either as a waste producer, transporter or manager, at the central offices of the Agència de Residus de Catalunya and the Territorial services of the Department of the Environment and Housing, upon payment of the corresponding fee
