There are four copies of the FA, an original and three copies, which are addressed to: the waste producer or holder, the waste manager, the town hall of the place where the waste manager is located and the Agència de Residus de Catalunya.

Each box of the document must be completed as follows:

The FA will be given to anyone who requests it representing any enterprise recorded in the database of the Agència de Residus de Catalunya, either as a waste producer, transporter or manager, at the central offices of the Agència de Residus de Catalunya and the Territorial services of the Department of the Environment and Housing, upon payment of the corresponding fee
